Payment for services is due at the time services are rendered. Exceptions to this policy are possible only when payment arrangements have been approved by our staff in advance or when we are participating providers with your insurance carrier (see the “Insurance” page). We accept cash, checks, MasterCard, Visa, Discover and American Express cards.
When fees are paid you will receive a “universal” insurance form with all the necessary information and signatures provided. You may attach it to your insurance form and submit it to your carrier for reimbursement. Prior to submission be sure that you have completely filled out the personal information on your form.
We are also CareCredit participants. This company provides financing, particularly convenient for short-term lending. Application is simple and, if this interests you, you should inquire with our front desk staff.
Returned checks and balances older than 30 days may be subject to additional collection fees.
