First Visit

Part of your initial appointment will consist of a consultation explaining your diagnosis and treatment options.

Often, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

Your surgical referral slip and any x-rays, if applicable
A complete medical history and list of medications you are presently taking
If you have medical or dental insurance, bring the necessary information or I.D. cards. This will save time and allow us to help you process any claims.

Online registration is available and encouraged. Completing the form is quick, secure, and will save considerable time for everyone. Please see the Patient Registration section.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

For patients undergoing intravenous anesthesia for surgery, please have nothing to eat or drink 6 hours prior to your surgery. You will also need an adult to drive you home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g., .diabetes, high blood pressure, artificial heart valves or joints, allergies, etc.) or if you are on any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.)

We pride ourselves on communicating effectively with you and in educating our patients to be able to make the right informed choices regarding care. Considerable information is conveyed prior to treatment and, for more involved problems, a second person (a relative or close friend) may prove to be a useful listener. Be assured that there will be ample time to have your questions and concerns addressed.
